Clinical Manager: Currently hiring a full time Registered Nurse Clinical Manager. The Clinical Manager works under the direct supervision of the Clinical Director and is responsible for the oversight of a clinical multi-disciplinary team, including the planning, controlling, organizing and leading of the team in the provision of effective and efficient clinical services. The Clinical Manager supervises, supports, and educates staff at the point of service in the field; supports an efficient and accurate revenue cycle by enhancing capture of appropriate revenue and managing the flow of documentation within their department to minimize days in accounts receivable and wll participate in administrative call rotation. Requires current license as RN and leadership experience is preferred.
New Hanover County:
Weekend Registered Nurse: Currently hiring an experienced RN to join our weekend team (Saturday/Sunday/Monday). This position is primarily responsible for the initial assessment and case development of patients in all stages of life in their homes based on the nurse’s experience and competency evaluation. Duties include but not limited to, assessing, interpreting, planning, implementing and evaluating patients according to the patient’s age and diagnosis. Must be a graduate of an accredited or approved school of nursing, either an AD, Diploma, or BSN program, license to practice in the state of NC, two years RN experience and a total of 2 or more years clinical experience is required. Supplemental experience may include experience as LPN, CNA, military medic, EMT or related experience. Home health experience preferred. Must have knowledge and comfort using computers, smart phones and reliable transportation.
LPN: Full time Licensed Practical Nurse (LPN) to make home health visits within New Hanover County. Prefer candidates with home health experience. Duties include, but are not limited to: traveling to and from patient homes, making 6-8 visits per day, documentation all visit notes within 24 hours of the visit, assist RNs with implementation of plan of care for all patients – all ages. Requirements: must be able to drive and have reliable transportation, be comfortable and familiar with electronic documentation, have availability Monday-Friday with hours from 8a-5p with an on call weekend rotation and have active LPN License in good standing.
Pediatric RN: Experienced pediatric RN to join our team as a full time Case Manager for our Pediatric population in New Hanover County. This RN Case Manager would be responsible for making between 5-7 patient visits each day. As an RN duties would included: assessing, interpreting, planning, implementing and evaluating patients according to the patient’s age and diagnosis, effectively and efficiently managing the care of a caseload of patients and coordinating care with a multidisciplinary team. Must be a graduate of an accredited or approved school of nursing, either an AD, Diploma, or BSN program, license to practice in the state of NC, two years RN experience and a total of 2 or more years clinical experience working in pediatrics is required. Home health experience preferred. Must have knowledge and comfort using computers, smart phones and reliable transportation. Well Care is 100% electronic Medical Records, experience on EMR preferred. The position requires availability Monday — Friday with weekly on call. *Must have recent experience working with pediatric patients, preferably in home health setting.*
Speech Language Pathologist: Well Care Home Health is currently looking for a full time Speech Language Pathologist/Therapist to join our therapy team. The ST performs patient evaluations and treatments according to procedural guidelines in the patient home; appropriately communicates and documents information in accordance to agency policies and procedures and discipline-specific guidelines; and demonstrates positive interpersonal relations in dealing with all members of the agency. This position is responsible for the speech therapy of patients in all stages of life in their home and will perform other duties as assigned. Home health experience preferred but not required. Must be a graduate of a clinical program from an accredited university or community college. Must have a master’s degree in speech-language pathology. Must be licensed in the state of North Carolina. Patient charting is 100% electronic; knowledge of ERM and computers required. Travel to patient homes throughout New Hanover, Onslow, Pender and Duplin counties required.

Occupational Therapist: We are hiring a full time OT to visit patients throughout Columbus and Bladen county, with overflow into additional areas as needed. Our OTs perform patient evaluations and treatments according to procedural guidelines and as ordered by physician. Home health experience preferred, but not required. Must be licensed on the NC Board of OT with 2 years experience. Availability Monday-Friday from 8a-5p is required. Supervision of OTA also required. Excellent benefits and option of company car!

Will travel to patient homes throughout the region and facilitate patient and family resources. Will be important part of a multi-disciplinary team or nurses and therapists. Must be a graduate of an accredited MSW program, with at least 2 years experience.
Psychiatric Registered Nurse: Full time opening for an experienced Registered Nurse who has worked recently providing services on an active treatment unit in either another home health capacity or psychiatric or mental health hospital. This Psych RN will visit 5-7 patients per day providing care. Must be proficient with electronic medical records as we are 100% EMR. Must have exceptional time management skills, availability Monday-Friday from 8a-5p.

Occupational Therapy Assistant: We hiring for a PRN/Casual Pool OTA to visit patients in Columbus and Bladen Counties. Our PRN staff work with a full time OT and are responsible for administering therapy treatments to patients in all stages of life as prescribed by their physician. Must have 1 year experience as an OTA, preferably in home health environment. Base office will be Whiteville. Must be licensed in the state of NC.
PRN/Casual Pool PT & RNs: We are hiring PRN/Casual Pool Physical Therapists and Registered Nurses to make weekend visits. Must have at least 2 years experience in your profession and be able to work Saturdays and Sundays. Travel to/from patient homes, assessing, interpreting, planning and implementing patient care. Must be a graduate from an approved/accredited school of nursing or physical therapy program. Must have professional license to visit patients in the state of North Carolina. Must have reliable transportation.
CNA (multiple openings): Certified nursing assistant positions available throughout Southeastern North Carolina. Will travel to patient homes and provide assistance with Activities of Daily Living including bathing, dressing, meal preparation, reminders, light housekeeping, medication reminders, etc. Time in the patient home will vary based on patient needs. Will follow the plan of care created by the Well Care nurse. Must have dependable transportation. We will work with you to find the perfect schedule.
