Lead Home Care RN: Well Care Home Care is seeking an experienced RN to join our team as a Lead RN/Clinical Manager. Responsibilities can include, but will not be limited to, managing the Private Duty Nursing Program, participating in hiring, coaching and training staff, facilitating excellent patient care and performing regularly supervisory visits with existing clinical employees. Past experience as PDN and/or In Home Aide Supervision preferred. Previous nursing management experience also preferred. Must be proficient in electronic medical records and documentation, possess exceptional time management skills, be very multi-task oriented and be able to work independently. The position is full time, Monday-Friday with competitive salary and benefits. Must have a current NC RN license.
Lead Intake Coordinator: Looking for a full time RN for our Lead Intake Coordinator position. This person is responsible for responding to inquiries about existing services and other resources in the community, initiate and facilitate the referral intake process for patients, facilitate the flow of patient information to and from staff, patients and physicians and manages the Intake Department workflow with a customer service and sales-centered focus. Additional responsibilities include collaborating with coordinators for disposition of referrals and communicating information according to department and company policies. Two years experience as an RN required. Home health experience preferred. Licensed to practice professional nursing in the state of NC. Prior experience in case management, discharge planning & community/physician relations preferred. Position is full time, Monday – Friday.
Temporary Clinical IT Support: Currently hiring temporary support to assist the day-to-day operations of home health help desk. Provides technical support distributing and maintaining laptops, smart phones, and Microsoft Office Suite and Horizon Homecare software. Will troubleshoot all help desk tickets and assign as appropriate. Provide initial and on-going computer training to al employees. Experience working in health care industry preferred, but not required. Extensive knowledge/background in IT required. Clinical (RN, LPN) background preferred, but not required, as this position is the primary resource for documentation navigation within Horizon Homecare software system. Position requires a five-month commitment with hours Monday-Friday from 8a-5p.
Clinical Manager: Rewarding and challenging position managing multi-disciplinary team of clinicians and clerical staff. Will facilitate excellent patient care, employee support, scheduling, accurate and timely reporting, documentation timeliness, staff competency, incident/concern investigations, etc. Must be very multi-task oriented and able to respond to staff at a moments notice. Will require advanced knowledge of EMR. Will participate in hiring, coaching, training and staff development. Will also implement disciplinary processes. Administrative on-call is required and may include evening or weekend visits should primary call staff be already on a visit. Must have several years nursing or therapy experience and 2 years experience as a manager of clinical personnel.
Home Health Certified Nursing Assistant: Currently hiring two full time Certified Nursing Assistants with at least 2 years experience. Will travel to patient homes and provide assistance with Activities of Daily Living including bathing, dressing, meal preparation, medication reminders, etc. Time in the patient home will vary based on patient needs. Will follow the plan of care created by the Well Care nurse. Must be experienced in home health. Must have reliable transportation and a valid CNA certificate in the state of North Carolina. Positions are in Onslow County and Columbus/Bladen Counties.
Occupational Therapy Assistant: Hiring a full time COTA to see patients in Columbus and Bladen Counties. Visit 5-7 patients per day in their home and provide therapy treatments per the patients individualized plan of care. Works under the supervision of a Clinical Manager and Occupational Therapist and perform tasks according to established policies, procedures, and guidelines as directed. Participates in professional improvement programs. Provides patient consultation and assumes as communicative role with other professionals. Performs other duties as assigned. Must be licensed OTA in North Carolina, have reliable transportation. Experience in home health with electronic medical records preferred.
Physical Therapy Assistant: We are currently looking for a full time Physical Therapy Assistants to join our team in Columbus and Bladen Counties. The PTA is responsible for administering prescribed PT treatments to patients in all stages of life, assisting with program development, communicating effectively and appropriately while following agency policies and procedures. Must have one year experience as a licensed PTA, proficient on computers and able to work Monday-Friday. Home health experience preferred, but not required Must be a licensed PTA in the state of NC.
Pender County RN: We are currently looking for an RN to work in Pender County. This RN will see 5-7 patients per day. Duties include but are not limited to, assessing, interpreting, planning, implementing and evaluating patients according to the patient’s age and diagnosis, effectively and efficiently managing the care of a caseload of patients and coordinating care with a multidisciplinary team. Must be a graduate of an accredited or approved school of nursing, either an AD, Diploma, or BSN program, license to practice in the state of NC, two years RN experience and a total of 2 or more years clinical experience is required. Home health experience preferred. Must have knowledge and comfort using computers and smart phones, have reliable transportation as well as exceptional time management skills.
Onslow County RN: We are currently looking for an RN to work in Onslow County. This RN will see 5-7 patients per day. Duties include but are not limited to, assessing, interpreting, planning, implementing and evaluating patients according to the patient’s age and diagnosis, effectively and efficiently managing the care of a caseload of patients and coordinating care with a multidisciplinary team. Weekend Baylor Registered Nurse: Well Care Home Health is currently looking for an experienced RN to join our weekend team (Friday/Saturday/Sunday). Duties include but not limited to, assessing, interpreting, planning, implementing and evaluating patients according to the patient’s age and diagnosis, effectively and efficiently managing the care of a caseload of patients and coordinating care with a multidisciplinary team. Must be a graduate of an accredited or approved school of nursing, either an AD, Diploma, or BSN program, license to practice in the state of NC, two years RN experience and a total of 2 or more years clinical experience is required. Supplemental experience may include experience as LPN, CNA, military medic, EMT or related experience. Home health experience preferred. Must have knowledge and comfort using computers, smart phones and reliable transportation.
Speech Therapy: Expanding our Speech Therapy Team with the addition of PRN or FT Speech Therapist. Position requires extensive travel throughout our region of care. The ST performs patient evaluations and treatments according to procedural guidelines in the patient home. Appropriately communicates and documents information in accordance to agency policies and procedures and discipline-specific guidelines. Demonstrates positive interpersonal relations in dealing with all members of the agency. The ST is responsible for the therapy of patients in all stages of life in their home and will perform other duties as assigned. Must have knowledge of Electronic Medical Records and prefer home health experience. Exceptional benefits. Must be a licensed ST in North Carolina.
