Occupational Therapist: full time and contract positions available. Join our dynamic rehab team in providing exceptional therapy in the home. Will treat diverse patient population with a high concentration of geriatric patients. Will work with other disciplines in creating a patient centered plan of care. Some supervision of OTAs when indicated for care. Company car for full time. Laptop, cell phone and equipment are all provided. Excellent salary with benefits.

Physical Therapist: Full time and contract positions available for licensed physical therapists in Pender, Onslow, Columbus, Bladen and Brunswick counties. Excellent rehab team! Will work closely with all disciplines to assist patients to their maximum potential rehab. It is very rewarding to see a patient become independent again in their home. Care Manager: dynamic RN or Therapist needed to lead clinical outcomes development, monitor utilization of services and implement clinical best practices. Will align closely with a team’s Clinical Manager to create excellent care at the patient’s bedside through care plan oversight and monitoring. Will lead weekly meetings, evaluate all new admissions, assist in discharge planning, monitor documentation and advocate for continuing education. This is a full time position requiring long periods of documentation review as well as constant phone interaction with the nurses and therapists seeing patients. Must handle frequent interruption well. Some travel required for meetings. Clinical leadership experience, Utilization Review and/or Home Health background preferred. Motivational skills a must.

RN Case Manager: Full time position for licensed RN. Seeking dynamic and organized RN to manage 30-35 patients in New Hanover County. Will develop a care plan specific to the patient and their diagnosis and then provide care in the home. Must be computer savvy, as we utilize electronic medical record. We will issue you a laptop and cell phone as well as all supplies needed to help patients meet their maximum independence. CNA (multiple openings): Certified nursing assistant positions available throughout Southeastern North Carolina. Will travel to patient homes and provide assistance with Activities of Daily Living including bathing, dressing, meal preparation, reminders, light housekeeping, medication reminders, etc. Time in the patient home will vary based on patient needs. Will follow the plan of care created by the Well Care nurse. Must have dependable transportation. We will work with you to find the perfect schedule. CNA II: Home Care visits for registered CNA II. Will travel to patient homes and provide assistance with ADLs according to a nursing plan of care. Human Resource Coordinator: Full time position to provide excellent people resources to growing and changing organization. Must have demonstrated success in recruiting and/or benefit management of an organization of 100 or more employees. Requires daily professional recruiting, extensive reporting, HR documentation and file maintenance, annual benefit enrollments and compliance, job fair participation, policy review and development, employee coaching, orientations and more. Frequent travel required. Occasional weekend availability for job fairs or Baylor orientations. Increased development of an electronic HR file is a priority in 2010, so must be very comfortable in all areas of Excel, Word and database type programs will be a priority.
