What is Home Health?
Home Health is the skilled medical and
support services provided by a professional
team of nurses, physical therapists, speech
language pathologists, occupational
therapists, medical social workers and home
health aides.This multi-disciplinary team
coordinates with the physician to provide
custom home care to meet each patient's
needs when it is necessary to help treat,
restore, rehabilitate or sustain a patient
in the home.
