Patient Screenings
Well Care Home Health is committed to improving the health of citizens residing in the communities we serve. The Centers for Disease Control (CDC), The Joint Commission for Accreditation of Healthcare Organizations, the Centers for Medicare and Medicaid Services (CMS), and the State Department for Health and Human Services are organizations that provide direction to our Agency in the form of expectations regarding quality of care.
In response to these expectations as well as a demonstration of our own commitment to excellence, Well Care provides two preventative health screening during the admission, resumption and re-certification time points. These screens are to assess an individual’s vulnerability to a fall and vulnerability for skin breakdown. Brief descriptions of both health screens are listed below.
Fall Risk Screen
Areas assessed are pre-determined and based upon National data and industry recognized expert sources it assesses your ability to walk, keep your balance, stand, sit, move from one area to the next, and evaluates the strength in your muscles. A score is provided reflecting the amount of risk which may make you more at risk of a fall. For scores at 10 or above, which reflect a moderate to high risk, Well Care Home Health will assign a Physical and / or Occupational Therapist, Skilled Nurse, or Medical Social Worker out to your home, based upon the scoring indicators to conduct a more thorough evaluation.
Skin Breakdown Screen
Areas assessed are pre-determined and based upon National data and industry recognized expert sources. It assesses your skin’s frailty pertaining to amount of moisture, degree of elasticity, response to friction and pressure, existing breaks in your skin, surfaces you sit/ sleep on and how you move from one surface to another. A score is provided reflecting the amount of risk you may have for skin breakdown. For scores at 16 or less, reflecting a moderate to high risk, Well Care Home Health will coordinate with Hill-Rom Company, Inc. to contact and inform you of medical equipment and / or products which would significantly aid in preventing skin breakdown.
For patients already experiencing a skin breakdown that is described by a clinician to be a Stage III or Stage IV, Well Care Home Health will assign a certified Wound Specialist from our Agency to evaluate. The results of that evaluation will be provided to your physician. These screenings provide beneficial information for you physician and Well Care Home Health clinicians, which will assist them in the development of a care plan, designed to focus upon your specific needs.
