    CAP / DA:

    Well Care Home Care is a state approved provider of CAP services. Case Managers throughout our service use WellCare to provide these home care aide services for home management and personal care.

    CAP/ C:

    The Community Alternatives Program for Children (CAP/C) provides cost-effective home care for medically fragile children (through age 18) who would otherwise require long-term hospital care or nursing facility care. The program contributes to the quality of life for the children and their families / caregivers. CAP/C services covered under the program include nursing services, case management, personal care services, some medical supplies, home mobility aids such as wheelchair ramps and respite care.

    CAP MR/DD:

    This program is a Medicaid funded program for persons with mental retardation and/ or developmental disabilities. WellCare offers community support, respite care, and enhanced personal care.

    The goals of the Community Alternatives Program for Persons with Mental Retardation / Developmental Disabilities

    • Address the needs of individuals in their community.
    • Ensure person-centered planning for each individual.
    • Provide for simplicity and ease of delivery of services.

    EPSDT:

    • Early and Periodic Screening and Diagnostic Testing for children under 21 years of age.
    • Personal Care Services.
