The Well Care rehabilitation team develops an individualized home rehabilitation program for each patient, providing the 360 of care your patients require. Physician specific protocols allow for a home program with a concentrated focus on the improvement of functional mobility, range of motion, stamina, and pain reduction.

PHYSICAL THERAPISTS FOCUS ON AREAS SUCH AS:
- Home safety and fall prevention
- Home based teaching for improved disease process management
- Pain management
- Anodyneâ Therapy: Infrared treatment modality for patients with longstanding peripheral neuropathies. Offering relief from chronic pain and poor circulation, this modality has been shown to decrease patients’ risk of falling.
OCCUPATIONAL THERAPISTS PROVIDE DIRECT INTERVENTIONS SUCH AS:
- Compensatory cognitive retraining
- Dressing/bathing skills and ADL adaptive training
- Home safety/assessment of adaptive equipment needs
SPEECH LANGUAGE PATHOLOGISTS PROVIDE RETRAINING AND REHABILITATION IN SUCH AREAS AS:
- Cognitive assessment and treatment
- Augmentative communication
- Dysphasia/hearing treatments
