A Look Inside Well Care's Cardiac Program

February is Heart Health Month, the perfect time to highlight the benefits of Well Care's Cardiac Program.

Our clinical teams across North Carolina are committed to providing heart failure and cardiothoracic surgical patients with the tools and resources necessary to bounce back to optimal health.

We want to keep you and your loved ones out of the hospital, and in the comfort of your own home!

Here are the four (4) main goals of Well Care's Cardiac Program:

1. Prevent re-hospitalizations.
2. Prevent Emergency Room visits.
3. Improve self care management.
4. Improve breathing and Activities of Daily Living (i.e. - bathing, dressing, grooming, etc.).

Our Cardiac Program is broken down into core features that allow us to accomplish these goals.

Here are 5 key features of Well Care's Cardiac program:

1) Customized Care Plan.
In collaboration with your physician, a Well Care Registered Nurse (RN) develops a care plan customized to fit your individual health care needs. The RN conducts a full head-to-toe assessment to evaluate your overall health, and then targets a specific area(s) for improvement. The RN partners with his/her clinical colleagues like Physical Therapists, Occupational Therapists, and/or Speech Therapists to ensure your ability to sustain normal activities and build stamina through tailored fitness programs. Our multidisciplinary clinicians work as a team precisely to keep you where you want to be - at home!

2) Telehealth monitoring and nursing oversight.
A specialized telehealth nurse oversees your vital signs (i.e. - blood pressure, pulse, and oxygen) using innovative technology, which helps to capture subtle changes to your health that may warrant further medical treatment. 

This technology combined with the nurse's ability to interpret the data and your vital signs is crucial! The key is to spot potential warning signs, with the ultimate goal of keeping you at home, and preventing a trip to the emergency room.

3) Patient Education.
Well Care's clinicians teach you and your families tactics to help you self manage your own care. Depending on your individual condition or needs, our skilled nurses and therapists outline specific actions through diet modification, medication changes, or customized exercise plans. These actions assist with identifying early symptoms.

4) Medication Management & Reconciliation.
Through collaboration with your pharmacist, Well Care's nurses help patients understand their medications, what they're used for, and how to use them properly. They also teach patients how to effectively organize and manage their medications. Read John Kabler's medication management blog post here! 5) Data sharing with patients' cardiologists.
Through the telehealth technology system, the Well Care team will keep you, your family and physician abreast of any areas of improvement, or decline, in your medical condition. Family members can also login and track your condition, if they'd like.
